The US Occupational Safety and Health Administration is investigating the cause of an incident in January at the Eglin US Air Force base - the country's largest Air Force base - in north-west Florida, which killed one man and injured three others when a large amount of firefighting foam was released into a hangar.
The fire suppression system in the 8,400 square metre maintenance hangar has 24 foam generators hanging from the ceiling. What caused the system to discharge the foam is yet to be ascertained, but a similar incident occurred at another Elgin US Air Force base hangar in 2012, which resulted in a foam blanket more than three metres deep.
Two operational F-15s were in the hangar when the foam was released, as were a non-operational F-15, F-16 and an A-10 used for training purposes.
